Before beginning any new exercise program, including one based on Form's recommendations, you should consult a qualified healthcare provider, especially if any of the following apply to you:
This list is not exhaustive. When in doubt, ask a qualified healthcare provider before starting.
Pain is a signal. If you experience sharp pain, sudden weakness, dizziness, chest discomfort, shortness of breath, nausea, or any symptom that feels wrong during a workout, stop the exercise immediately and seek medical attention as appropriate. Do not attempt to "push through" symptoms that concern you based on the app's recommendations.
Form includes an injury-tracking feature that lets you tell your coach about injuries or limitations. The coach will attempt to route around those injuries in generated plans — but the feature is not a substitute for professional rehabilitation guidance. If you are recovering from an injury, work with a qualified physical therapist or sports-medicine professional. Tell Form about the injury so the AI can adapt, but verify any adapted plan against your clinician's advice.
Form can display heart rate, heart rate variability (HRV), and a daily readiness score derived from Apple HealthKit data and your subjective inputs. These numbers are intended to inform your training decisions, not to diagnose or monitor medical conditions. If you have concerns about your heart, nervous system, or general health, consult a qualified healthcare provider; do not rely on Form's readiness score as medical data.
Form may discuss nutrition, body weight, and body composition in the context of fitness goals. These discussions are general information, not medical or nutritional advice. For individualized guidance, work with a registered dietitian or qualified nutrition professional, especially if you have a history of eating disorders, diabetes, or other conditions affected by diet.
Form is not an emergency service. If you are experiencing a medical emergency, call your local emergency number (911 in the United States) or go to the nearest emergency room. Do not use Form's chat to seek emergency help.
By using the Service, you acknowledge that you have read and understood this Medical Disclaimer, that you accept full responsibility for your own health and safety during any activities suggested by the Service, and that Cartwheel Form LLC is not liable for injuries, health outcomes, or other consequences arising from your use of Form's fitness and coaching content.
